Student Registration


Student Registration
Parents should collect the following items and contact the appropriate school secretary listed below.  All forms must be in English.
  1. Registration Form - completed and signed by parent or legal guardian.
  2. Official custody documents will be required if child does not reside with both parents.
  3. Child's original birth certificate with raised seal.
  4. Medical record form (Kindergarten/new USA residents only) with required immunization records - dated, stamped and signed by child's doctor.
  5. Proofs of residency:
    1. Original deed or original closing statement (or legal contract showing purchase of home), AND
    2. Any two current utility bills with both name and address included (i.e., PSEG, cable, water, tax bill from Bergenfield tax office), OR
    3. Legal lease/rental agreement with both names of parents and children on it (Landlord Statement Form), AND
    4. Any two current utility bills with both names and address included (i.e., PSEG, cable, water)
  6. Parent affidavit of legal residence and pupil domicile completed and notarized (Affidavit Form).
  7. Picture ID of one of the parents (if they have a passport, driver license)
  8. Transfer card and immunization record from previous school
  9. Transcripts from previous school
